Youth Case Manager

Lake Region Human Service Center - Devils Lake - Youth Case Manager Be part of the solution to the behavioral health crisis. As a team member for ND Department of Health & Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover.As a Youth Case Manager, you will work within an interdisciplinary team to help meet the needs of youth and families. You will engage with youth and families and provide orientation to the services that are offered/recommended. You will coordinate services with outside agencies, including other behavioral health providers, probation, schools (including attending school meetings), etc., and will connect regularly with parents/custodians in regard to the child's needs. You will conduct comprehensive needs assessments, complete and implement service plans, and leverage contacts and referral partners to ensure access to needed services. Case managers may also assist with crisis response when appropriate. Case Management services are community-based. Work will be done primarily in the individual's home and community. To thrive in this position, you should be knowledgeable about resources offered in your community and have a strong desire to work with youth and families in their homes and natural environments.To be considered for the Case Manager position, you must meet one of the following requirements: a bachelor's degree and two years of experience working with individuals who have behavioral health conditions in a facility setting or a home and community-based location.OR a master's degree OR have at least five years of paid supervised experience working with individuals with SMI/SED in a role with case management functions such as individual assessment, care plan development and maintenance, referral and appointment scheduling, monitoring and follow-up activities.This position requires a valid driver's license and successful completion of the assigned defensive driving course during new-hire orientation.About HHS: ND HHS strives to make North Dakota the healthiest state in the nation by reinforcing the foundations of well-being: physical, economic, and behavioral.
